Transitional Living Program

The Wedgwood Transitional Living Program offers safe, stable and nurturing homes for youth, ages 14-17, who are unable to live with their families or in a community home setting. Transitional Living provides home-based intervention therapy that assists youth in gaining independent living skills in a safe and supportive environment. Youth will work toward gaining the necessary skills to succeed in a variety of settings. This may include a family foster care placement, family reunification, or a transition to independence or an independent living program.

Transitional Living is dedicated in assisting youth with education and healthcare. Some services include:

  • High school graduation
  • College entrance
  • Entering the workforce
  • Counseling
  • Healthy lifestyles
  • Life Skills

Mohammed was the primary caretaker for his sick single mother since he was eight years old. His much older brothers struggled with drug and alcohol addiction. Mohammed would pay the bills by finding back-breaking labor.

After his mother’s death when Mohammed was 17, his older brother kicked him out, leaving him homeless. Even though he was living in the park, he managed to finish the 11th grade. He told a teacher about being homeless and the counselor connected him with ACH. After being at the Youth Emergency Shelter, he was able to move to the Wedgwood Transitional Living Program and finish high school.

Mohammed is now living independently in an apartment and working a full-time job. He plans to pursue higher education in Psychology.
